Funds will be used for the design, development and introduction of three new products into world markets:

• Manufactured Foundation Components: Fab-Form has recently rented 3,500 ft2 of adjacent warehouse space for the manufacture of foundation components to be sold into the Vancouver housing and condo markets. The patented monopour system allows for premanufactured foundation form components to adapt to uneven ground by the adjustable side supports and flexible Fastfoot footing formwork. Once local manufacturing has been rationalized, it is Fab-Form's intention to franchise this opportunity to truss plants across North America.
• Fast-Tube CLEAR Column Form: Fab-Form has recently re-designed and newly woven the fabric for the Fast-Tube column form. The new fabric is clear for visibily of the concrete pour, stronger and stretches less. The installation guide and logo is printed on the outside of the fabric. It is Fab-Form's intention to have major construction supply houses distribute this product around the globe.
• ZAT Catwalk Bracket: Fab-Form introduced the Zont Bracing System into the ICF industry three years ago. The system has been very successful and is now the largest selling ICF brace in the world. Customers have requested a catwalk bracket which we have recently developed and proven in the Vancouver market. It is Fab-Form's intention to market this product to ICF users around the globe. This requires testing to ensure the bracket meets OSHA and other jurisdictions safety standards.

Completing the Subscription Agreement

All subscribers must fill in the personal information on pages A2, A3, A4 and A5. In addition they must choose an exemption listed on pages B5 and B6. There are five exemption categories:

1. Accredited Investor: Your net income exceeded $200,000 in each of the most recent two calendar years or you own more than $1million in cash and securities or your net assets exceed $5 million:
   • Complete page F2 and Schedule 1 of Form 1
2. Friends, Family and Business Associates: The placee is must fall into one of the eleven categories listed on page F4:
   • Choose category on page F4;
   • Complete Schedule 2, Form 1;
   • If from Saskatchewan, complete Form 3 on page F13;
3. Minimum Investment of $150,000
   • Complete page F5;
4. Employee, Executive Officer, Director or Consultant
   • Complete page F5;
5. Existing Shareholder (as of 31 October 2017): Under BC 45-534, current shareholders can participate up to $15,000. Similar legislation occurs for other provinces across Canada.
   • Complete page F-5.
6. USA Investor
   • Complete Form 4 located on page F14 - F17
